If you have a question about any of our services, general physical therapy questions, comments or suggestions, please send them to us through our on-line form.
If you are a new patient, or feel you may need physical therapy, please utilize our printable forms below.
Prescription form. If you think you are a candidate for physical therapy, print out the prescription form and bring it to your provider so that you may be referred to Albany Physical Therapy for treatment.
Appointment instructions. Once you are referred to Albany Physical Therapy, print the appointment form containing all the information you need for making your first appointment.
Registration form. After your first appointment is made, you can print the registration form, fill out the information ahead of time, and bring the completed form with you to your first visit.
Albany Physical Therapy is conveniently located at 4 Executive Park Drive (behind Stuyvesant Plaza) in Albany, the second building on the right as you turn onto Executive Park Drive. There is ample free parking and easy access from I-87, I-90, and Central or Western Avenues in Albany. Our office is located on the first floor, with a handicapped-accessible sliding glass door entry.
